LOOKING: file uploader

I am looking for a PHP based file uploader. It would need to have a few text fields and a file upload field..

Example:

Name:
Email address:

Browse for file upload

Let me explain better

I want my customers to be able to fill in a form that contains Name, Email, and have a little description box of the file they are uploading and of course, a upload field.

I been on Hotscripts and tried everything they had. I found either a emailable form or upload file script.. I need the 2 in one... thanks a lot fellas!
