Before install I had categories, forums and sub forums on my forum index page:

Logged in or not, all I have now is Recent Threads:

How can I get my categories to show up on my forum index page like they did with normal VB install?

Looks like you've replaced your vBulletin index.php file with the CMPS. You'll need to get your original vB index.php file back, and then follow the instructions for installing the CMPS in the same folder as vB from the link below.

That made sense and they are both now visible at their absolute URL’s, however they are not combined on the index (home page).

It almost seems this would have to be done using mod’s to make it look like I want.
I made the picture, of my desired look, below from screen shots.
Please give me your thoughts.

If you don't need a separate CMPS page like your forums/index.php file, then you would pretty much need to un-do the steps you just did with renaming the files, and then just use the Integration feature.

(be sure to enter 'index' in the THIS_SCRIPT Integration Method section instead of integrating your index.php file.

I have started a new site. It is a clean install. I want the layout to look like the picture above. I will change other site after I make this on work.
OK I have done all you have suggested above (integration), however there is no forums on any pages, nor can I post any threads. There is one forum (Sports I created) and the main cat that came with install.
I have tried to add a mod to include forum but, I cannot see the forumdisplay.php file to be listed in the Add Module -> PHP File Module -> Main Options -> PHP File Module Options -> File to Include. I am not really sure how to handle this anyway.
Please explain how I can get my layout it to look like picture above.
