"Welcome Module" Log-in Issue

I can not Log-in using the "Welcome Block" - when I submit username and password and click log-in, it merely erases my log-in entry. But, I can log in via the Navbar log-in.

I tried the cookie clearing solution and it is no help. Also tried adding ".mydomain.com" in cookie domian under cookie options.

Please assist.

Do you have a test account that I can try logging in with?
