Default v3.0 RC1 Hacked?

There is a vB forum on my server that is using CMPS v3.0 RC1 as their main page. For the second time they have had iframes inserted into their main index page. How could this be happening? The account password is a very secure password so I know that is not the issue. No other files have been touched except for the main vbadvanced index page and the inserted iframe causes the site to redirect to another one.

The only thing I can figure is they have their permissions for their index.php file set to 777 and someone is altering the file from another script. There is no code in the CMPS though that alters any files on the server or even any code that inserts data to the database.
